Procedure Supply List

Items needed for THORACENTESIS:

  • 1 Kendal Thoracentesis tray from central supply (the "Kendal" part is very important, but if they are out of stock, use the Arrow tray - DO NOT use the Cardinal or Care Fusion trays!!)
  • 2 liters worth of vacuum bottle WITH tubing
  • 1 disposable scalpal
  • 6cc of 1% lidocaine without epi
  • Table in room for patient to lean on w/ pillow
  • INR, platelets if indicated
  • Sterile syringe cap if you are sending your big syringe down for cytology
  • Consent form signed
  • Orders in EPIC for fluid labs

Items needed for ARTERIAL LINES:

  • Arrow Arterial line kit
  • Table w/ 2 chucks or towel, tape to secure wrist
  • A-line setup apparatus from Aurora
  • Sterile Gloves
  • Consent form

Items needed for PARACENTESIS:

  • 1 Kendal Thoracentesis tray from central supply (yes, it is a thora tray, and the "Kendal" part is very important, but if they are out of stock, use the Arrow Thoracentesis tray - DO NOT use the Cardinal or Care Fusion trays!!)
  • How every many liters worth of vacuum bottles you will need WITH tubing
  • 1 disposable #11 blade scalpal
  • 6cc of 1% lidocaine without epi
  • IV access if planning to remove large amount of fluid or need for albumin replacement
    • Rule of thumb is 50cc of 25% albumin for every 2 liters of fluid removed if you are taking large volumes (>4L) or have renal/BP issues.
  • Sterile syringe cap if you are sending your big syringe down for cytology
  • Sterile gloves (Tierney = sz 7)
  • Consent form signed
  • Orders in EPIC for peritoneal fluid labs

Items needed for SKIN BIOPSY:

Items to order from supplies ahead of time:

  • Surgical Skin BiopsyTray with Prep Kit (includes lidocaine and injection needle/syringe)
  • 4mm disposable skin biopsy punch - from surgery control desk (Ph# 863-4237, item iventory # p425) - yes, you need this one in ADDITION to the biopsy tray
  • Specimen bottles (one per biopsy) with saline or formalin depending on what you are looking for (contact pathology and ask how they would like to receive the specimen). Cultures can't be done off specimens sent in formalin.
  • Histology request carbon copy form from histology
  • Suture (4-0, 5-0, or 6-0 nylon depending on site of biopsy and size of punch you are taking. Usually don't need suture if using <5mm punch)
  • Needle driver, iris scissors, and forceps for suturing
  • Know what part of lesion, which age of lesion, which body location to obtain bx

Stuff you can get yourself on the station:

  • isopropyl alcohol swabs, providone-iodine, or chlorhexidine to prep site
  • extra 25g needle to lift the punch biopsy out of the skin with
  • Sterile gauze pack
  • Bandage
  • Consent form signed
  • Orders in EPIC for path as "Gross and Microscopic Surgical Panel" for each biopsy done
  • Surgical panel must be collected by the nurse on the station prior to delivering the specimen to histo WITH the histo request paper form

Items needed for CENTRAL LINES:

  • Triple lumen catheter pack (20cm catheter for most) **If pt may need ct pulmonary angio or rapid contrast bolus, a "Pressure injectable" triple lumen should be placed (stored on station 20)
  • Towel pack – sterile
  • Sterile gown, mask, hat, gloves
  • Ultrasound, if available with sterile probe cover
  • 3 quick blue hub caps with saline flushes in 3 separate syringes
  • CVP setup at bedside if needed
  • Line cart in room
  • INR, plts if indicated
  • Consent form signed

Items needed for LUMBAR PUNCTURE:

  • Sterile Lumbar Puncture Kit (contains a 20g 3.5" cutting spinal needle)
  • 22g, 3.5" spinal needle (if patient has good landmarks, this smaller whitacre/atraumatic needle reduces the spinal headache rates significantly) - order two needles
  • 6cc of 1% lidocaine without epi
  • Large packet of betadine with or without the swabs in it
  • chart verification of no heparin (therpeutic or proph), no plavix for 5 days, no aggrenox, INR <1.5. ASA is ok.
  • Consent form signed
  • Orders in EPIC for fluid labs

Items needed for ARTHROCENTESIS:

  • Skin permanent marker
  • Sterile gloves
  • Sterile fenestrated drape with aperture or sterile 5-towel pack to make sterile field
  • One 5-mL syringe for lidocaine injection with a 25g anesthetic needle and a 22g anesthetic needle
  • 1% lidocaine w/o epi (2 cc)
  • 18g x 1.5" needle for joint aspiration
  • One or two large 30-60cc syringes for joint aspiration depending on amount of fluid in the joint
  • chloraprep wand
  • Several 3x3 or 4x4 gauze packets
  • Sterile bandage
  • Consent form signed
  • Orders in EPIC for synovial fluid labs
